Megan Shreve Nominated for Health Care Hero Award
RISE is pleased to announce the finalists for the 2024 RISE Health Care Hero Award. Among
the distinguished nominees, Megan Shreve, CEO of South Central Community Action Programs
(SCCAP), has been selected as one of the five finalists for this prestigious honor.
The Health Care Hero Award is presented annually at The RISE Summit on Social
Determinants of Health, recognizing individuals who have made significant contributions to
improving the lives of under resourced populations through healthcare and social services
interventions. These individuals exemplify the RISE mission to promote health equity among all
patients.
RISE received an overwhelming response this year, with 59 national nominations submitted for
consideration. The finalists were selected based on their exceptional dedication, innovation, and
impact in addressing social determinants of health, and improving healthcare access and
outcomes for vulnerable communities.
Megan Shreve's nomination highlights her exemplary leadership and transformative impact. Her
dedication to addressing crucial social determinants of health, including poverty alleviation,
housing and food security, and work at the micro and macro level, have earned her recognition
and admiration from peers and community members alike. In her tenure as CEO spanning two
decades, Megan has shown a deep understanding of the intricate relationship between social
services and health outcomes. Her innovative approach to community engagement, especially
in coaching other sites across Pennsylvania, has spread best practices far beyond her
immediate sphere of influence. These efforts reflect her unwavering commitment to creating a
system of social care that supports all families in achieving economic mobility and whole family
wellbeing.
The winner of the 2024 RISE Health Care Hero Award will be announced at The RISE Summit
on Social Determinants of Health in New Orleans, where healthcare leaders and innovators
gather to share insights and strategies for advancing social care and health equity.
